Genetic conditions contribute significantly to morbidity and mortality but are often overlooked because they are considered rare. Whilst referred to as ‘rare diseases’, genetic conditions may be individually rare, but they are collectively common. In South Africa, 1 in 15 births is said to be affected by a congenital disorder of which 80.5% are genetic, or partially genetic, in nature. Genetic conditions involve all organs, and the pancreas is no exception.

Autoimmune pancreatitis (AIP) is a rare condition and does not commonly present a clinical problem to the average HPB clinician. It is, however, important to correctly identify this disease and make the diagnosis for three main reasons:
· It often mimics, and is misdiagnosed as, pancreatic cancer
· It usually responds well to steroid therapy
· It is often related to other autoimmune conditions
